I found the problem. Database corruption.
To re-cap the investigation which may help someone in the future:
I restarted XTension and the log showed:
5/19/08 7:01:23 PM XTension startup version 6.0 beta 2 (build 652)

5/19/08 7:01:23 PM 6 Global Scripts Loaded.

5/19/08 7:01:24 PM Missed Event: Hourly Routine
5/19/08 7:01:24 PM 4 Events Loaded
5/19/08 7:01:25 PM XTension (ppc) Ready. Registered to: Mark Johannessen
5/19/08 7:01:25 PM Leopard or newer detected. Using UT16 encoded apple
event string replies.
5/19/08 7:01:25 PM XTension was not shutdown properly.

Everything looked fine.
I then opened all of the interface status windows and pressed a
wireless keypad button. The CM15a Rx turned green and then dimmed -
correctly registering an RF command. I then pushed the button a
number of times and nothing. Then, after a wait - a bunch of hits
came through. Then I hit the button and all hits came through until
nothing. All this time, nothing from the W800. And also, there was
no activity in the log of these hits (preferences logs database items
only) except an occasional "Received Ninja command: UNKNOWN Ninja
command (3F) for housecode O from CM-15a". I then waited awhile and
the CM15 registered receiving every button push, and then stopped
again.
The CM15a debug log was:

Then XTension crashed and the processor fan on my G5 hit high gear.
After restarting Xtension, I got this:

5/19/08 8:03:29 PM CM15: CM15 - Make sure CM15.kext is in
/System/Library/Extensions.
5/19/08 8:03:29 PM CM15: No CM15 Found
5/19/08 8:03:29 PM CM15: Error opening CM15 (1)
5/19/08 8:03:29 PM CM15: Problem opening the device
5/19/08 8:03:29 PM Interface CM-15a has been unexpectedly disconnected
from XTension.
5/19/08 8:03:34 PM Retry connection to interface "CM-15a". Tries left: 98
And then dialog "The application cm15.isf quit unexpectedly" appeared.
I then renamed the database file and restarted Xtension. The
interfaces began to work again.
I went into XTension Database [Show Contents] Contents/Preferences
and deleted the InterfaceArray key, array and dict references and
restarted XTension. I added the interfaces again and both the W800
and CM15 recognized the RF keypad signals without delay. I then had
to re-assign the interfaces to every X10 and wireless unit.
All is good again.
