On-demand printers

I designed a paperback book in a square format. Two months ago, my client placed the order with BookSurge. All along, he planned to "reprocess" it as a hardback to distribute to a small number of donors and benefactors. But, lo and behold, BookSurge became CreateSpace and, along the way, jettisoned the hardback option altogether. And we checked with Lulu. They only offer two sized of hardbacks--letter and 6x9--neither of which fit the current layout.

Are there any other on-line on-demand printers that do casebound books with custom trim sizes? In this case, the tie-in with a retailer like Amazon is not necessary, as they will be privately distributed.
