Designing instructional and didactic books

Anyone have any recommendations for such books? I am doing more and
more books specifically written to teach from, not merely as
"academic" books to be used in college-level courses. I'd like to
pick up some guidance on such things as

line spaces between paragraphs
use of itals and bold for special words
sidebars
bulleted and numbered lists
use of images, diagrams, etc.
general page size, text line length, typesize, etc.

I'm feeling that my long background in general book design doesn't
stretch to elucidate the special problems in textbooks and other
didactic texts.
